Fitness Facilities Information & Forms

If you are interested in applying for a membership to use Doane's fitness facilities, please read the Haddix Center and Campus Recreational Facility Hours and Policies document and follow the steps below to complete the process:

STEP 1:  Fill Out Forms Appropriate for Your Type of Membership.

Regular Memberships: print off, complete and sign the forms below



  1. Haddix Application - Community Member, and
  2. Waiver of Liability and Hold Harmless Community Member



                                           DOANE EMPLOYEE DEPENDENTS:

  1. Haddix Application - EE Dependent, and
  2. Waiver of Liability and Haddix Hold Harmless EE Dependent

STEP 2:  Payment, Form Submission and Access IDs. 

  1. Bring your completed application forms, payment, and a photo ID to Doane's Business Office (located in the lower level of the Padour Walker Building) during regular business (8 - 4:30 pm Monday - Friday).  Current rates are $180 for individual membership and $300 for family membership.  As of 1/1/22 rates will increase to $190 for individual membership and $350 for family membership.
  2. Once your application and payment are complete, go to the Doane Safety Office (in the upper level of the Padour Walker Building) to have your photo ID card issued.  This card will be required for each member to access the Haddix fitness facility or any other Doane fitness facility. 

Not Ready for a Regular Membership? 

Daily passes are also available for $5 per day.  Please pay at the Business Office located in the Padour Walker building.
