Athletic Forms

Doane College student-athletes need to have athletic health forms completed and returned each year to the Doane Athletic Training Staff.  A student-athlete can not participate in practices or games until the required forms are completed and returned.  If there are any questions, you can contact Greg Seier, Head Athletic Trainer, at 402-826-8500 or greg.seier@doane.edu.
